Application forms should be filled neatly and legible in blue or black ball pen only.
Incomplete/unsigned application without required enclosures are liable to be rejected without any prior notice to the candidate.
SPECIFIC INSTRUCTIONS:------
Course applied : Tick in the box for course you are applying in.
Name of Candidate : Name must be written in both Hindi and English(capital letters) as given in 10th Class or equivalent Certificate.
Date of birth : Write the date, month and year as per English Calendar, as given in 10th class or equivalent certificate.
Age : As on 31st December of the year of admission is to be written.
Gender, Nationality, Religion and Marital status should be mentioned.
Category must be denoted.
Write the father's name/husband's guardian's name in capital letters.
Academic Qualification : Candidates are required to fill up the qualification details from 10th class onwards.
Address : Candidate should fill up permanent address & correspondence address with phone number/mobile number for communication, in the space provided in the form.
Declaration : The candidate must sign at the end of form, without which the application will be rejected.
Photograph of candidate : Paste (do not staple) recent (not more than 6 months old), clearly visible and colored photograph on the space provided in an application form, admit card and hostel form.
Signature of candidate : Signature should be done in provided box.
SUBMISSION OF APPLICATION FORM----------
Duly filled in application form should be submitted with Rs. 100/- either in cash or through D.D. for entrance examination in favor of
"SIR ASHUTOSH TECHNOLOGICAL RESEARCH INSTITUTE" payable at SODEPUR by any Nationalized Bank.
ADMISSION ELIGIBILITY:-----------
Minimum & Maximum age for admission will be 17-35 years respectively.
For ECG, PHYSIOTHERAPY, DENTAL, ALLOPATHY(PHYSICIAN ASSISTANT), HOMEOPATHY(DHMS), there is no age limit.
Minimum Qualification is H.S
Student should be medically fit.
ADMISSION RULES AND REGULATION:--------
College will not allow the candidate for any Installment delay
Institute has right to reject the application form or admission of candidate at any stage, if the information furnished by the candidate is found to be incomplete/ incorrect information.
No fees is refundable if candidate withdraws his/her admission at any time after joining the course.
Candidate should bring all original certificates with 1 set photocopy, original T.C.,original residential certificate,medical fit certificate,1 copy adhar or voter Id, 6 passport sized recent colour photographs, admit card and ADMISSION fee at the time of counseling and admission.
Candidate must bring first installment fee in the form of CASH or Banker's Cheque in favor of 'SIR ASHUTOSH TECHNOLOGICAL RESEARCH INSTITUTE” AT SODEPUR by any nationalized bank at the time of admission.
Candidate should follow rules and regulations of the college and should maintain discipline.
Candidates have to submit prescribed amount in the Institute for issue of APPRON.
